Method of Participation
|
There are no fees for participating and receiving CME credit for this activity. During the period December 16th, 2009 through December 16th, 2010, participants must 1) read the learning objectives and faculty disclosures; 2) participate in the educational activity; 3) print and complete the downloadable evaluation form; and 4) submit by fax to Postgraduate Institute for Medicine at the end of the activity, at (303) 790-4876. A statement of credit will be issued only upon receipt of a completed activity evaluation form. Your statement of credit will be mailed to you within 3 weeks.
